چکیده

In everyday practice, clinicians are faced with questions on the comparative benefits and risks of the various health interventions available to treat patients. When performed well and reported completely, randomized controlled trials (and systematic reviews of such trials) are generally considered to be the most scientifically rigorous approach to determine whether a cause-effect relationship exists between an intervention and a given outcome. However, clinical trials are hampered by their tendency to focus on simultaneous comparison of no more than 2 or 3 alternatives (eg, treatment vs no treatment, treatment with option A vs B and A vs C). To make more informed decisions, knowledge of the existing evidence on the numerous alternatives available in clinical practice, as well as quantitative recapitulation of this information, is often required to estimate the comparative efficacy and safety of health interventions. Ideally, approach would require clinical trials with as many treatment arms as there are available alternatives, allowing diverse comparisons to be made between option A and B, B and C, C and D, and so forth, and not only the determination of whether A is simply better (or not worse) than B. This information would also be useful for guiding clinical decision making through the use of systematic reviews and clinical practice guidelines that stringently evaluate the evidence and clearly state which are the best choices for treating a specific patient. However, the information required is often only partially available and/or has major associated uncertainties. Limited resources and the need to determine in relative terms the beneficial or potentially harmful effects of multiple treatments have meant that, consciously or not, the transitive property is always present in clinical reasoning when choosing a treatment, in the planning and design of a new clinical trial, or, more recently,

Evaluation of task complexity with using the number of practice trails

The purpose of this study was to explore the effectiveness of the number of practice trials in determining the degree of task complexity. Sixty subjects according to the types of tasks (A-B) and receiving feedback (100%, omitted, self-control) were randomly divided into six groups. All of the subjects practiced 100 trials tasks in each day on first two days. Retention tests were performed during...

The transitive fallacy for randomized trials: If A bests B and B bests C in separate trials, is A better than C?

BACKGROUND If intervention A bests B in one randomized trial, and B bests C in another randomized trial, can one conclude that A is better than C? The problem was motivated by the planning of a randomized trial, where A is spiral-CT screening, B is x-ray screening, and C is no screening. On its surface, this would appear to be a straightforward application of the transitive principle of logic. ...

Blindness in Randomized Controlled Trials

In combination with randomization, blinding or masking is an important factor in randomized controlled trials (RCTs), particularly in trials that assess therapeutic effects. Here an attempt is made to explain blindness and why it is important. In clinical trials, blinding is defined as the condition imposed on a study in which study participants, health care providers and assessors collecting o...

بهبود پروتکل AODV جهت مقابله با حملات کرم‌چاله در شبکه‌های اقتضایی

Mobile Ad hoc Networks (MANET) are vulnerable to both active and passive attacks. The wormhole attack is one of the most severe security attacks in wireless ad hoc networks, an attack that can be mounted on a wide range of wireless network protocols without compromising any cryptographic quantity or network node.  In Wormhole attacks, one malicious node tunnels packets from its location to the ...

Counterexamples to the Transitivity of Better Than

Ethicists and economists commonly assume that if A is all things considered better than B, and B is all things considered better than C, then A is all things considered better than C. Call this principle Transitivity. Although it has great conceptual, intuitive, and empirical appeal, I will argue against it. Some philosophers believe that the concept of better than implies Transitivity, so Tran...
